Today is Mark Souder's last day as a congressman. The moralistic Indiana Republican, one of the most vocal drug warriors in the House, this week announced he was resigning due to "a mutual relationship with a part-time member of my staff" (the same staff member who worked with him on a video urging sexual abstinence). Some highlights from Souder's career:

• He introduced legislation that denied college aid to students convicted of any drug offense, including marijuana possession. Bank robbers and murderers, meanwhile, were still eligible.
• He likened consumption of politically incorrect intoxicants to rape and child abuse.
• He called medical marijuana "just a phony excuse to be a pothead" and staunchly opposed legislation that would have stopped federal raids on medical marijuana suppliers.
• He opposed the nomination of former Massachusetts Gov. William Weld as ambassador to Mexico, citing Weld's support for medical marijuana.
• He was a major author of the law that aimed to suppress illicit methamphetamine production by imposing nationwide restrictions on purchases of allergy and cold pills containing pseudoephedrine.

Souder also co-sponsored the Internet Gambling Prohibition Act and the Military Honor and Decency Act, which would ban Playboy sales on military bases.

In November 2008, Radley Balko hoped voters would finish Souder off. Balko noted a 2007 appearance on Tucker Carlson's MSNBC show in which Souder defended increased funding for the government's demonstrably ineffective anti-pot propaganda on the grounds that "a marijuana user is very seldom just a casual marijuana user."

But the headline puzzles me: wasn't Souder's meth precursor bill a good thing, reversing Reagan administration's failure to stop the trade at the behest of pharma interests? And hasn't that bill stopped the proliferation of meth labs in this country? (I fear perhaps, because it has moved to Mexico and contributed to the violence there.)

The FTC outlawed us getting the "free promotional goodies." Samples are important, as some folks cannot afford medications consistently. Samples are also a cost saving measure if latent allergy is present (always a risk in taking any medication). Why spend a lot of money on a new and expensive drug if it is hazardous and even deadly to your health? Since it is private business supplying the product, I see no issue with dispensing samples.

You bring up a great point though and my biggest issue with Big Pharma is their sucking government cock to keep their protectionist measures in place. Big Pharma fears competition, big time!

I don't have any issue with prescribing either one. The only legal issue is that MJ is scheduled I, which is fallacious in itself as medical benefit is demonstrable. I have railed on these boards before about the conflation of "addiction" and "drug tolerance and dependance", and they are NOT THE SAME. The larger problem is cultural, as "illicit" drugs of abuse are viewed as a health hazard of riff-raff as opposed to legitimate personal lifestyle choices. It is incumbent on the physician to listen to his or her patient and assess PX accurately so effective PX management therapy can be started. Since I am not the patient, I have to assume they know their PX better than I do. However, their is the phenomenon of "drug seekers": those that game the system to obtain drugs to sell, particularly in ER's. The reason I have a problem with this is, because of my license, I am liable for what I prescribe and the contract I make with a patient is "I prescribed this for YOU. Not for anyone else. The contract is between you and me, and part of that contract is you don't sell this to other people for their personal use." Again, it addresses my liability, which is a legitimate concern regarding practicing medicine.

btw, i recently had surgery and my physician prescribed me oxycontin and percocet, as well as muscle relaxers.

i've discussed this with several physicians and they FEAR the DEA, and rightly so. oxy, as a CII substance means that DEA strictly scrutinize scripts and also script patters (iow, what is the average dosage over time the MD prescribes per patient), etc. and looks for unusual patterns

drug seekers exist. i deal with them all the time, but the point is that dr's should not be dissuaded from prescribing pain meds to patients out of the fear they MIGHT be drug seekers. it's better to let 5 drug seekers get their meds, if by being too suspicious, it means that LEGIT patient doesn't get it. if the drug seeker OD's, that's his friggin fault.

the benefit is that, while it is very orally bioavailable, rectal absorption is excellent, and it avoids (somewhat) first pass liver metabolism. i've seen kids do enemas with grain alcohol for the same effect.

you couldn't do it effectively with vicodin or percocets, since by weight they are overwhelmingly APAP, and that would interfere with the absorption of the opoid.

the street term is "plugging".

Correct, Poston. You get an A for explaining the rectal route of administration of suppositories. However, APAP suppositories are an effective route of administration and distribution of a bioavailable drug specifically because of the abundance of blood via a thin mucous membrane and sanguine tissue. Vics or Percs probably won't get you high rectally unless you shove a large amount of them into the rectum. But then age and health and drug tolerance must be taken in as a factor. It should also be noted the max dose of APAP is 4 grams/24h for an adult.

when you are talking 5mg of opioid (weak, in the case of hydrocodone) and around 600 mg of APAP, the problem is that there is only so much surface area, etc. essentially, they risk plugging their plug, so to speak, with all that APAP. the hardcore user is going to need AT LEAST 20 mg of hydro to get much effect, and that's 2 frigging grams or so of APAP. it is simply not a good way to go.

Vicodin is obviously an easy prescription to get. A few years ago I had minor oral surgery, the doc removed a small growth from my lower inner lip. Local anesthetic, took 10 minutes. The doctor asked me if I wanted a prescription for pain killers, I said "sure, why not". She prescribed thirty Vicodins! Frankly, I didn't need even one.

I filled the prescription, and took one later that day. It worked! I don't remember the dose, but it was pleasant. I didn't take any more after the one because I was worried about the 650mg acetaminophen part of the pill. Overdosing on Tylenol will kill you. The other 29 pills sat in my medicine cabinet for years before I finally pitched them.

once the liver damage is done, it's irreversible.

even so, hydrocodone is about 1/2 as strong mg per mg as oxycodone. it's a weak opioid.

APAP suicides are messy. One of the primary functions of the liver is to produce clotting factor. Non-functional liver == no clotting factor and and hemotoxicity. living in L.A., I think that most people have scripts for casual use. That's not to say there isn't a medical benefit. A city doesn't support 700 (or whatever the number is these days) dispensaries based on medicinal usage alone.
