Age May Trump Vaccination Status in Assessing COVID-19 Risk

Risk calculations must move beyond vaccinated/unvaccinated, age data suggests. Media coverage of the COVID-19 pandemic these days tends to focus a lot on outcome variance between vaccinated and unvaccinated people, gatherings, and locales. Meanwhile, we may be overlooking the continued salience of age in predicting risk and disease severity.
But "in assessing an individual's risk of dying from covid, age appears still as important—and maybe even more important—than vaccination status,"
:After 18 months of public-health guidance promoting universal vigilance, I think hardly any American has a clear view of just how dramatic these differentials are. All else being equal, an unvaccinated 66-year old is about 30 times more likely to die, given a confirmed case, than an unvaccinated 36-year-old, and someone over 85 is over 10,000 times more at risk of dying than a child under 10. And although many infections still go undetected (complicating any attempt at a universal calculation of risk), your chances of dying from a confirmed case roughly double with every five to eight years of age, as countless data across multiple countries demonstrated last year — an effect larger than even the most significant comorbidities. The "exponential growth" of mortality risk by age is, in other words, another aspect of the pandemic we have processed only poorly.
Over the past nine months, vaccination has utterly transformed the shape of the pandemic in the places where it has penetrated the whole population. The effect isn't just visible in countries like Portugal or Iceland — where the threat appears to be fast receding and which give an encouraging picture of our possible future — but in parts of the United States as well. But for all its transformative, liberating power, vaccination has not broken the basic age skew of the disease or offered anyone an exit ramp from it. Instead, in two profound ways, vaccination has confirmed the age skew: by producing severe breakthrough cases concentrated overwhelmingly in the elderly and by reducing the risk faced by individuals by an astonishing degree that is nevertheless smaller than the still more striking effect of age.
In the days of the delta variant, breakthrough cases among the vaccinated are pretty common. But they remain overwhelmingly mild in younger vaccinated populations.
Only 14 percent of deaths in vaccinated COVID-19 patients occur in people under age 65, according to the Centers for Disease Control and Prevention.
A Financial Times analysis found the COVID-19 mortality risk is about equal for vaccinated 80-year-olds and unvaccinated 50-year-olds, while an unvaccinated 30-year-old has less chance of dying than a vaccinated 45-year-old.
"It's a sign of just how large the age skew is to begin with that getting vaccinated doesn't deliver you into an entirely new category of pandemic safety — safer and more protected than anyone who hasn't gotten vaccinated — but simply pushes you down the slope of mortality risk by the equivalent of a few decades,"

Over the course of the entire pandemic, which has killed more than 135,000 Brits, just one boy and seven girls between the ages of 5 and 9 have died; between the ages of 10 and 14, nine girls and five boys have died. These are all tragedies — and each means many more years of life lost than with a death among the elderly — but they are nevertheless relatively few in number. As schools reopened on the backslope of the U.K.'s Delta surge, there were about seven times as many British kids under age 5 hospitalized with the respiratory disease RSV as there were with COVID.
This is not to say that unvaccinated children face absolutely no risk from COVID, given that many millions of Americans under the age of 18 have gotten sick, and almost 500 have died, over the course of the pandemic. It's just that the risk those 73 million minors do face is — relative to the risks faced by their parents and grandparents — very, very small.
Numbers like these should influence the way we think about the risk to different populations. They also show how our calculations must extend beyond simply whether people are vaccinated or not.
"To believe that the vaccinated elderly are now perfectly safe, as can be tempting to all of us who are desperate for the unvaccinated to get with the program, is to raise an uncomfortable set of questions about the way we have processed risk by universalizing it," Wallace-Wells concludes:
If we want to believe, say, a vaccinated 75-year-old is safe, have we now simply normalized a higher level of individual risk than seemed moral to accept as recently as 12 months ago, given that they may not be any less in danger of dying than an unvaccinated 53-year-old? If we are now debating what we can do, in schools especially, to protect unvaccinated children, who are much safer still, should we not be discussing at the same time what measures can be taken, beyond boosters, to protect the vaccinated elderly? Mask wearing offers differential benefits, too: according to the much-applauded study in Bangladesh, cloth masks of the kind typically worn by children offer very little protection, and the strongest effects of surgical masks were observed among the elderly.

No. I am saying that taking the vaccine resolves the risks for yourself. What is NOT included in a risk calculation is the risk to everyone else once the ICU's are full of people being treated for covid.
Taking the vaccine resolves both risks even if coincidentally because the vaccinated are not filling up the hospitals with covid treatment. Not taking the vaccine DOES transfer the risk of a hospitalization to others in the event your infection happens in synch with other infections (which tends to happen). And my solution - don't allow the unvax to be treated for covid in ICU if the ICU's are full - does in fact resolve that risk to others. By making that particular risk a risk to oneself instead of a risk one can foist off to others.

JFree, what is the purpose of a hospital if not to treat those that are sick or injured? Even those illnesses and injuries that are completely preventable. Virtually every action you take increases your chances of going to the hospital. What makes COVID unique?
Hospitals have had 18 months to prepare for COVID surges. That was the original intent behind 2 weeks to flatten the curve. Hospitals plan their resources around the conditions in their area. At what point does the blame for a full ICU shift from the unvaccinated to the hospital for mismanaging resources or even to the government for having too restrictive regulations not allowing for proper use of resources? Having said that, I would still say that hospitals have a right to manage (or mismanage) their resources any way they choose because they own said resources. These do not belong to the community to be doled out only to the worthy.

What makes COVID unique?
It is contagious. Meaning that it presents a significant risk to both staff and other patients. It is in epidemic mode right now. Meaning it is currently filling the ICU's and forcing rationing. Those patients tend to fill up ICU for quite a long time. Meaning that each ICU bed filled by a covid patient blocks 2,3,4,+ other ICU patients from getting treatment. And actions to prevent hospitalization have a near-immediate effect unlike most other preventive medicine. Meaning that unlike smoking/weight one can take action now [rather than in the past] - to hugely prevent hospitalization within a couple weeks [in time to affect medical burden during an epidemic that is burdening medical] rather than a couple years [when it doesn't matter].

No they haven't because, in 2021, said preparation depends entirely on the decision by people unknown to the hospital to not vax. Hospitals have been desperately trying to get people to vax. That is the only preparation they can do. If everyone vaxxed, there is no ICU burden in this country. Period.
People are making the decision to not-vax with the assumption that their risk of death is low because they WILL get into the hospital if things go wrong. But that latter risk is exactly the problem that happens when the medical system is overwhelmed. If they do not get into hospital if things go wrong, then their risk of death is actually near identical to the risk of hospitalization. That latter is the death risk that the unvax should be assuming for themselves in making their decision to vax because they should not be assuming they will fill up the hospitals and force everyone else out. You can see the risk difference - and it is huge.

And my proposal doesn't kick in until ICU's are full and care is already being rationed. But for states where there is/was a problem - it's real fucking easy to see - eg Alabama. Then ICU hits max [which first forces emergencies to not be admitted - more dead people]. Only very slowly can the problem of leaving enough capacity to deal with heart attacks, stroke, car accidents be resolved. In the meantime - yet more people die - eg Ray Martin DeMonia who died of a heart attack (in Mississippi - 200 miles from his home) because the 43 ICU's in Alabama that were nearer were all full.

What was the point of two weeks to flatten the curve if hospitals have no way of preparing for COVID outside of hoping people get vaccinated? For example, the hospital in my small community set up an entire wing with the sole purpose of handling COVID surges. They have been stocking it with the exact treatments necessary for handling COVID efficiently. Sitting around and hoping that people get vaccinated so they don't end up at the hospital is the worst business plan possible. You are trying so hard to lay all of the blame at the feet of the unvaccinated that you are blind to everything else. I think all of this is more showing that the government needs to get out of the healthcare industry. They have made it too cumbersome, costly and difficult for hospitals to promptly respond to the needs of the community. This goes well beyond the COVID pandemic.
Also, I am not sure you are aware, but there are many other preventable contagious deadly diseases that will land you in the hospital. COVID is not unique.
Now, I will agree with you that nobody should be making the assumption that they will be given immediate access to someone else's resources. That is not a good personal risk assessment.

This is to say that the chart isn't rate of infection per 100K vaxed 40-49 YOs vs. 100K unvaxed, but the overall rate of infection (~2000 per 100K) split according to vaccine status.
Looks like the vaccinated make up some 55% of the age 40-49 case load, but considering the age skews on vaccines you’d expect them to make up 85%+ of that cohort’s population.

I have always realized that the COVID19 was an opportunist infection that had greatest impact on those most stressed. So the fact that death were higher in older individuals has always made sense. Likewise the fact that individual in poor heath do to lifestyle (overweight) or circumstance (cancer patient) are also at greatest risk of significant health impacts or death.
What I think we may actually be missing is the middle ground where individuals are significantly impacted but not so bad they are hospitalized. I have talked to people who did get sick and reported in to be very unpleasant experience. I have also talked to employers complaining that individuals contracting the illness are out for several weeks leaving them short handed. Worse the contagious nature of the disease often means that several employees will be out either all at once or sequentially. Overall this means the loss of work and school time.

I got checked once after the fever set in because I was exhausted. But once I got a prescription for cough syrup with codeine, I could sleep without waking up and immediately felt better. The lowest my blood oxygen ever got was 92, in the waiting room, because I was wearing a mask. Yes, I'll say it, masks are worse than COVID for healthy breathing.

McConnel is playing this the right way to maximize the chances of the $3.5 trillion budget reconciliation bill falling apart. He deserves credit for that!

As you well know, "default" is a very technical term of art when it comes to organizational finance and debt securities. Would the US actually be in default on any of those existing obligations? I genuinely don't know. But I don't see such hypothetical purchases as qualitatively different than what the Fed has been doing the last decade or so.

Nothing new there. We have known all along that the risk of serious illness or death increases with age. We have also known that the vaccines are not 100% effective. Nobody informed has claimed otherwise.
I think there is too much focus on the death rate. It is far from the only bad outcome. Even if it doesn’t land you in the hospital it can knock you on your butt for weeks or months and the acute stage can feel like absolute crap. Some people I know have had lingering symptoms of fatigue and brain fog for months.
Other than general trends I have stopped looking at the numbers.
On the good news front both Pfizer and Merck are in late stage trials for new oral drugs to treat Covid. The idea is you would start taking it as soon as you test positive and then for a week. They are finding that it can at least cut duration and symptoms in half.
Also the home test kits are available but hard to find. People are grabbing them as soon as they come in. I have one on the shelf and used one when I felt sick a few weeks ago. Negative.
What's the treatment basis behind those oral drugs? How long does something like take to be OTC?
The one I saw looked like it is a biologic an immunoglobulin based drug. Remdesivir is but that has to be given IV. I doubt it will be OTC but it would be nice if it could be dispensed by a pharmacist.
