Free the Meds: 5 Drugs You Should Be Able To Buy Over the Counter
The FDA is unnecessarily making your life more difficult.

The Food and Drug Administration (FDA) approves not only which drugs can go to market but also whether they require a prescription from a doctor. The agency gets this power from the Durham-Humphrey Amendment of 1951. Meant to protect patients, the law has turned into a license to make life difficult. Here are five low-risk drugs that the FDA could make available over the counter today.

Albuterol: Research shows that albuterol is vastly superior at relieving asthma symptoms compared to other inhalers, which is why it's offered over the counter in Singapore, Australia, Slovenia, and South Africa. It's also available in the United Kingdom under the name Salbutamol. There is no logical safety-related reason why albuterol sits behind the counter while its competitors, the epinephrine drugs Primatene Mist and Asthmanefrin, are available without a prescription.
Insulin: Two outdated versions of insulin are sold over the counter because they were grandfathered in after the passage of the Durham-Humphrey Amendment, but those aren't the products that today's diabetics want or need. As insulin prices have skyrocketed in the U.S., a black market for insulin has emerged. Products like insulin can and should be sold through online retailers like Amazon, especially given the long wait times to see the medical specialists who prescribe the drug.
Tobramycin: Tobramycin eye drops are used to treat conjunctivitis, more commonly known as "pinkeye." It's pretty obvious when someone has pinkeye, but patients still have to go to the doctor to get a prescription for these unharmful eye drops. That often means parents either have to go to the emergency room or miss work to take their kid to the pediatrician.
Simvastatin: Heart disease is the leading cause of death in the U.S., and statins—cholesterol-lowering drugs—can reduce the risk of a heart attack and other common deaths from heart disease. Simvastatin (and other statins like lovastatin) are prescribed for cardiovascular disease. This particular drug has been over-the-counter in the U.K. since 2004 but has failed to get approval for unrestricted usage in the United States. A 2016 study published in the American Journal of Managed Care estimated that over-the-counter statin access could prevent over 250,000 major coronary events over 10 years.
Birth control: With over 9 million users, daily-use oral contraception, or the birth control pill, is the most common form of contraception in the United States. The American College of Obstetricians and Gynecologists, which represents birth control prescribers, says it "supports over-the-counter access to hormonal contraception without age restrictions." On July 13, the FDA did approve the progestin-only Opill brand for over-the-counter use, which is a step in the right direction.

A whole range of vaccines have been immunized against lawsuits ever since ambulance chasers nearly shut down vaccine production back in the 80s. In exchange for immunity, drug companies pay a per-dose tax to fund a government program that compensates anyone injured by those vaccines.
There's a very similar program in place for COVID vaccines, precisely because people expected this kind of bullshit and didn't want unreasonable suits slowing down development and deployment in the midst of a global pandemic. Pfizer (and Moderna, J&J, etc) all pay into a fund that offers compensation for people injured by their vaccines. So why do lying hysterics try to make it sound like people are just left out in the cold?

The wording of the FFDCA is such that one reason a drug or device can require a prescription is not that the article itself is dangerous or difficult to use, but that making it OTC means people can skip a visit to the doctor when they should be seeing one. For instance, the reason given for albuterol's being prescription only is that asthma is not a condition for which self-care is appropriate in an advanced country. Authorities would like to get the OTC drugs adrenaline (inhaler) and ephedrine off the market too, but have been thwarted by the standards for removal of old drugs from interstate commerce as being difficult to satisfy.
It's a work protection racket for doctors. It's like, what the hell, this is a rich country, people can afford to see a doctor for chronic needs like asthma, birth control, etc. They can see a nurse practitioner instead, but only if a medical doctor lets them.
Why is self care for asthma not appropriate in an advanced country? I would think we are *better* equipped to manage it ourselves here.

Because the idealists in charge of our lives want us all to get the most expensive and sophisticated "help" we can get. Asthma? They're thinking they should all be on antiinflammatories and/or mast cell inhibitors and being monitored by a specialist at frequent intervals as their lives and circumstances change.
Because it should be monitored in case it gets worse, so a doctor can change the treatment, rather than the patient just going through more inhalers.
Almost everything primary care physicians do these days could be replaced by an app. It's a job that continues to exist only because of government protection.
Tobramycin, and every other anti-biotic, should NEVER be available over-the-counter. It will rapidly become ineffective due to the creation of resistant bacteria by its use. It is already over used by physicians.
Insulin should likewise not be over the counter. It is a very difficult drug to use properly, even by physicians. Over use will cause death.

Yeah, "right dosage" between "dose to take without poisoning themselves" and "dose to effectively prevent a coronary artery event" the answer is "they won't". Something like 99+% of people, even with a doctor's help and routine monitoring, don't hit that mark.
Wikipedia says that over-the-counter can refer to both of those. Perhaps your use is more precise but the article's use isn't wrong.
And maybe having to talk to the pharmacist isn't a bad idea for some of those. Or maybe we just need prescriptions that last 10 years if your condition isn't going away anytime soon. Nobody should be taking insulin if they haven't ever been diagnosed.

Doctor's aren't perfect, but I think its probably better that they be the ones to prescribe insulin. I suspect self-diagnosis will lead to a lot more people in hospital beds with hypoglycemia.

Statins too. I take a statin as of a year ago. I’m really unsure how my doctor worked out the proper dosage. And if I can’t sort that out on my own easily, vey few other lay people will.

Cholesterol imbalance is a very common issue among men from my father’s side of the family. I likely will not be able to manage it without medication going forward. Until my father, every male on his Sid elf the fami,y was dead before 60. If something else didn’t get them, they all died of a heart attack or stroke. At least over the last 200 or so.
And again, I would not have been able to work out the dosage on my own. I’m sure it was set by the results of my unmedicated blood work, plus other factors.
Regular A1-C readings combined with regular readings of kidney chemicals will give your Doctor a much better indicator of how much is too much. Again, it varies from patient to patient.

Ideologically, I agree with you. Practically, I don't think the social infrastructure or culture is there to support it. Kinda hand-in-hand with more general drug abuse and legalization. I think drugs, including harder drugs, should be legal, but there has to be a realization that in many, many minds the onus will be on you to either plan for or rationalize the mountain of bodies that will pile up and "I'm a libertarian." won't cut it any more than "They were social degenerates/inferiors."
What mountain of bodies? Legalization might lead to slightly more use, but I suspect the increase will be both small and mostly transitory. If anything, legalization would likely lead to a lot fewer injuries and deaths. If you were going to pop some pills or do a little blow, which would you prefer? Something cooked up in someone's garage by a guy with a chemistry degree from the University of my Cellmate? Tobramycin use is unnecessary and wasteful.

Symptoms are easily, effectively, and cheaply treated with clean moist compresses and plain tylenol.

Pink eye isn't even one illness, it has many different causes. Trying to treat it with an antibiotic when it is caused by something else can be quite dangerous.
I’m all for all of these being Over-The-Counter and Off-The-Shelf, though they should also include warnings on possible side effects.
Not every body reacts the same way to statins, for instance, as my Rhabdomyolysis will attest. It took 7 years for statins to get to me, whereas my Sister’s used them for 18 years with no ill effects. Fortunately, my Doctor wisely took me off of all of the statins.
Metformin needs to be in candy dispensers and Mounjaro needs to be much more available and inexpensive as well! Those are miracle drugs from my end and for many other people too!
Insulin needs to be pumping out of vat-grown pancreases and flowing like water until Medicine can advance to transplanting pancreases and/or steel-plated artificial pancreases!
Also, perhaps if there weren’t so damn many taxes and regulations and government didn’t pay farmers to not grow crops, people’s blood pressure wouldn’t be through the roof and they wouldn’t need statins.

Speaking to Kan News on Wedneday, Yael Rotenberg and Maya Desiatnik expressed their anger over the IDF's failure to heed their repeated reports of the ominous activity. Compounding their rage, the two were the only Combat Intelligence Corps soldiers who survived the Hamas attack on their base near the Nahal Oz kibbutz, less than a half-mile southeast of the Gaza border.
“It’s infuriating. We saw what was happening, we told them about it, and we were the ones who were murdered,” said Desiatnik.
The types of activity they observed at the border was concerning enough, but, even more strikingly, the pace had steadily intensified as Oct 7 approached. Desiatnik said it proceeded from weekly, to daily, to almost nonstop. In addition to her general visual observations, she said she was able to obtain information about the substance of the training, which encompassed driving tanks to bypassing the fence via tunnels.

Two other IDF intelligence soldiers shared similar experiences with Israel's Channel 12.

These four IDF soldiers aren't the only ones who say their warnings were ignored. But they underestimated such warnings,” an anonymous Egyptian official told the Associated Press.

Once Reason figures out the actual list of criteria for making drugs OTC vs prescription only, then we can have an informed discussion on this. Reason authors generally don't understand that a libertarian society generally has more restrictive rules than the society we currently have; libertarianism is about who imposes the rules, not about the presence or absence of rules.
When it comes to drugs, in a libertarian society, drugs would likely be as unavailable to most people as they are now. First, many drug manufacturers would impose restrictions for fear of liability. Second, insurers would impose strong rules against self-medication and deny coverage/impose penalties if you took unapproved OTC drugs.

I agree these (and most) drugs should be available OTC, but I wouldn’t trust insulin from an online retailer. Insulin needs to be stored below 48 degrees but cannot be frozen. The likelihood of spoiling the insulin by allowing it to warm up or by freezing it with a bunch of Below Zero ice packs is high. Larger shipments to the pharmacy can have temperature “shock watch” tags on the shipment to detect temperature variations but the home delivery insulin (at least the ones we’ve tried) have no such protection.

While I agree with most of the above I think that eye infections (conjunctivitis) should be seen so the treatment is specific for the type of infection involved. An eye infection due to Beta Strep can be very very serious and a tetracycline like drug is not appropriate or necessarily effective treatment. Making Tobramycin over the counter will delay necessary treatment.
