America Needs More Dentists

America doesn't have enough dentists, and Ahmed El Zawi could be part of the solution—if only the federal government would let him.
El Zawi emigrated from Libya to attend the University of Washington's dental school. He graduated in March 2017 and is licensed to practice dentistry in Washington state and Virginia. But it is illegal for El Zawi to work as a dentist—to work at all, actually—because his student visa expired in March. Though he found an employer willing to sponsor him for an H-1B, his name was not selected in the annual lottery that determines who receives one of those valuable government permission slips.
El Zawi was able to renew his student visa to stay in the country legally. He's taking business classes while biding his time until next year's H-1B lottery, when he plans to apply again. "Right now I'm spending money and not earning money," he says. "I would much rather be working."
We should want him working as a dentist, too. As of December 2017, there were more than 62 million Americans living in what the U.S. Department of Health and Human Services (HHS) defines as Dental Health Care Professional Shortage Areas—census tracts with fewer than one dentist per 5,000 residents. The department estimates that an additional 10,800 dentists would be required to eliminate the shortages completely. And the problem is getting worse: In 2013, the number of Americans living in areas with inadequate access to dental care was 45 million.
Because demand vastly exceeds supply, dentists can take their pick of locations and clients. They tend to cluster in relatively wealthy suburbs, and as a result, the shortages identified by HHS are overwhelmingly in inner cities and rural areas.
There's a class divide as well. According to a 2014 survey by the American Dental Association, only about one-third of dentists accept patients on public insurance programs such as Medicaid.
The Pew Charitable Trusts estimate that more than 800,000 Americans were treated in emergency departments in 2009 for problems related to tooth decay. All those E.R. visits cost the health care system more than $110 million.
The H-1B visa process allows no more than 85,000 visas to be awarded each year, but 190,098 applications were received in 2018, according to U.S. Citizenship and Immigration Services. The caps are politically designed to limit how many high-skilled immigrants can come to the country to compete with Americans for jobs.
But when it comes to dentistry, there is no need for protectionism. In fact, protecting American interests would actually mean importing as many non-U.S. residents as we can.

You dentist never thinks about the kreb's cycle. Neither does your orthopedic surgeon, ER doc, or neurologist. All of them passed some exam about the kreb's cycle at some point.
They nearly never think of the Pythagorean theorem either. Triangles, angles, distance, stress, fluid dynamics, metabolism, biochemistry, all of those need to have an intuitive understanding. You do not get that without practice.

The H1B visa program is dumb. By slowing in more highly skilled professional immigrants, the country would in effect benefit from a brain drain. Instead the push is to let in just the low skilled ones such as the caravan participants. It's a typically dumb way to function.

Dental cleanings are the one service you can't do for yourself these days. Otherwise you could basically never leave your home or at least never have to talk to anyone. #hermitlife
And yet a relatively low skilled dental hygienist does all the work, the dentist looks at your teeth for 2 minutes and says come back in 6 months. Do we really need more than one dentist per 5000 people? Or do we really need more low cost teeth cleaners, and only dentists for the really tough jobs?

It is a direct result of government raising the costs of other health cares faster then the rate of inflation, and letting dental care (and vision care too) fall into the ranks of no insurance, or poor insurance. And shockingly, people don't buy dental care when government actions have swallowed up all their health care dollars, and a few more with mandates for poor health care insurance.
i had a cracked tooth that needed a root canal a few months back and I would have signed over my house to the dentist I was in so much pain. People will buy dental care at any price when they need dental care as there are few things more painful.

But limits on foreign medical professionals have little to do with immigration caps, and more to do with AMA/ADA lobbying. Even with open borders, the AMA/ADA would ensure that foreign-educated medical professionals practicing in the US would be severely limited, with the full support of Democrats.

The dentist does have options while awaiting a green card. He can find a fellowship, residency or research position in a non profit hospital. Also there are government jobs at the VA or other government contracted positions which are exempt from the restriction.
The problem is not professional licensing. If he jumped through all the hoops the states will give him a license. He cannot work because of federal visa requirements.
We do need more dentists. The government should be relaxing restrictions for student and H1-b instead it is tightening them. Kinda like how paying more for steel makes us more secure.
Just a personal note on a 45 year career of H1B visa workers in the computer world, specifically mainframe application development and maintenance. I never worked with an H1B holder who could honestly be described as 'highly skilled and trained/educated'. They were about the same as the Americans working side by side with them. Mostly grunt line coders; with a few who had better than average English skills and helped the others 'understand' the system specifications.
But they did have some common characteristics; the consulting company that actually employed them charged a lot less than the other consulting companies, and held their passports, and payed them a lot less than the Americans.

Dentistry has got to be the biggest racket in the USA. Although my Norwegian relatives told me they have to go to Estonia or Hungary to afford serious dental work. I've solved all such conundrums by implementing one of my three life hacks: never live more than a 2-hr. drive from an international border.
I talked to a guy from Florida once who said he takes his family to Honduras for major dental work.
Since way back in the 70s, I used to go to MEXICO for all my dental work and even medical operations too. It is so cheap in Mexico, that it would cover my airfare from PA and MS and leave money for a vacation too.
Now that I live here in Mexico, I still often use paid doctors, labs and clinics, who offer low cost and fast service, even though as a retiree, I get it FREE from the slow, bureaucratic govt. service, IMSS:
I rely on surgeons, dentists and doctors in Curitiba, Brazil, where it is almost always 61?F. AirBNB, airmiles and Uber make it reasonably simple for anyone with a passport to fly to the most European place in South America and benefit from excellent medical care the like of which These States used to have long ago. An open airline ticket is the best and cheapest insurance money can buy. Freelance interpreters plentiful, and rare is the doctor who doesn't know English.

With enough dental hygienists, you have less need for actual dentists, because people don't get so many dental problems.
There isn't really a good reason why getting your teeth cleaned should be more involved than getting your hair cut, or a pedicure.

I'm not sure why we would lower the requirements for foreign dentists while leaving the requirements in place for domestic dentists. Seems like you're doing it wrong if this is your solution.

It was the same argument regarding primary care physician shortages, and they missed that point as well. These are all supply issues. Yet somehow, they also failed to notice that expanding access to already rare good has predictable results.

Given that fact, why don't we only let in the highly skilled, high income ones, who actually pay in more in taxes than they use in government services?
Then we could import as many dentists, and doctors, and engineers as we could ever want. Permanently, not even on temporary visas. Just a thought guys...
I do not see Dr. El Zawi as an import.
He went to an American dental school. He completed all requirements and holds two state licenses. All he wants to do is work.
His problem has nothing to do with professional licenses. He has them.
Our immigration system is fractured by intention.
Where did I say anything about professional license being an issue here?
My point is that we allow in hundreds of thousands of useless immigrants with no educations every year... This sucks up a lot of slots that can/should be going to people like him. He IS a foreigner. He was a foreign student. That he got his education here doesn't really come into play. If he'd graduated in India my point would be the same.
We've worn out Americans patience for immigration with the sheer numbers of them... And the problem is too many of them are low skilled ones that don't bring anything to the table. They should all be people like this.
If you can only fit 5 passengers in your car at a time, which is kind of how immigration works unless you want to completely overwhelm a nation, you want all those passengers to be doctors and engineers, you don't want to fill 4 spots with smelly hobos and then not have room for the brain surgeon.
This is essentially our problem right now. Too many spots are filled by janitors and dish washers, but we don't want EVEN MORE total immigration... So we end up not allowing in good folks because we've filled ourselves to capacity with too many bums.

Yes definitely we need more high quality dentist. But we should also low down the treatment cost. And also it is pretty hard to find the Best Dental Office.
