Theresa Frazier died twice in a five-month span. Well, she didn't really die. The Canadian government just mistakenly declared the 77-year-old woman dead twice. Her problems started when a woman with the same name passed away, and the government somehow thought that it was her and cut off her government pension. Frazier thought she'd resolved the problem only to have the government declare her dead a second time.
