HHS Blames Trump Policy for 180,000 Unplanned Pregnancies

Dubious assumptions underlie estimate of unplanned pregnancies caused by Trump policy. "The Biden administration is working to reverse a Trump-era family policy directive that…officials believe may have resulted in an estimated 180,000 unplanned pregnancies," reports Newsweek. The startling estimate comes from a new proposal from the U.S. Department of Health and Human Services (HHS) to rescind the Trump-era rule. But to get this unintended pregnancy estimate, the health agency had to make some pretty big leaps in logic.

The biggest flaw with the HHS estimate is that it assumes a lot of people not receiving family-planning services subsidized by a particular federal government program in 2019 simply went without any birth control or family-planning measures at all.

The funding in question comes via Title X of the Public Health Service Act, under which the federal government can "make grants to and enter into contracts with public or nonprofit private entities to assist in the establishment and operation of voluntary family planning projects."

In 2019, the Trump administration finalized a rule saying that no Title X funding could go to health clinics or programs that also referred women to abortion services or provided information about abortion. (The money was already barred from going toward the direct provision of abortions.) "Litigation over the 2019 rule immediately ensued," notes the HHS proposal. "The Department was sued by 23 states, every major medical organization, Title X grantee organizations, and individual grantees." But appeals courts upheld the Trump administration rule.

As a result, 19 out of 90 recipients of Title X grant funding dropped out of the program, according to HHS. "Overall, the Title X program lost more than 1,000 service sites. Those service sites represented approximately one quarter of all Title X-funded sites in 2019."

With fewer groups getting Title X grants, fewer patients were serviced under the auspices of Title X money. "With the 2019 Final Rule only being in place for five and a half months, the remaining 71 Title X grantees served 844,083 fewer clients as compared to the previous year," HHS reports.

HHS does not say these service sites—many of them Planned Parenthood clinics—shut down, and there was no news in 2019 of a wave of Planned Parenthood clinic closures or masses of people being turned away from Planned Parenthood services. (Searching Google for 2019, the only news of Planned Parenthood clinics closing is that two in Cincinnati, Ohio, closed after facing threats that included state rule changes. "These closures are the result of years attacks on our ability to provide reproductive health care," Planned Parenthood said in a statement to ABC News.)

Presumably, Planned Parenthood—whose lobbying arm spent $45 million on the 2020 election—largely found other ways to cover budget deficits left by lack of Title X funds.

Planned Parenthood itself does not report serving fewer patients or helping prevent drastically fewer unintended pregnancies. Its annual reports say that for 2019–2020, it served 2.4 million patients and that an estimated "395,000 unintended pregnancies [were] averted by contraceptive services
" it provided. And for 2018–2019, it also served 2.4 million patients, and "approximately 393,000 unintended pregnancies [were] averted by Planned Parenthood contraceptive services."

In addition, groups still receiving Title X money were given more of it in 2019, which should have enabled them to serve more patients. "Fifty existing grantees were awarded $33.7 million to expand Title X services" and seven states "had a meaningful increase in the number of Title X clinics in their states," HHS says.

There's no reason to suspect that patients whose visits were previously subsidized by Title X money at Planned Parenthood (or other clinics that left the grant program) simply gave up on birth control or other family-planning methods altogether. Yet that's exactly what HHS assumes to get its unintended pregnancy increase estimate.

The new HHS proposal gives a lot of data on how many fewer people were served using Title X money: "Compared to 2018, 225,688 fewer clients received oral contraceptives; 49,803 fewer clients received hormonal implants; and 86,008 fewer clients received IUDs." But we don't know that people went without these things entirely, merely that clinics weren't using Title X grant money to pay for them.

Nonetheless, the agency extrapolates from there that "as a result of the decrease in clients able to receive Title X services, it is estimated that the 2019 Final Rule may have led to up to 181,477 unintended pregnancies."

It arrived at that number by "estimating that of the 844,083 fewer clients served by Title X in 2019 compared to 2018, 21.5% of those clients could have experienced an unintended pregnancy as a result of not receiving services." HHS said it came to this number from a formula developed by the Guttmacher Institute, a reproductive rights research group.

The Guttmacher Institute said in a 2017 paper that "the formula for calculating pregnancies averted among general populations of publicly funded family planning clients is to multiply the number of clients by 21.5%, to get the number of unintended pregnancies averted."

However, using this to estimate the number of unplanned pregnancies caused by fewer patients being served by Title X grants does not take into account that these patients may have been served by other publicly funded (or otherwise subsidized) planning programs.

Adam Toledo dropped gun, got killed by cops anyway. Body camera footage released by the Chicago Police Department on Thursday gives a fuller picture of the March 29 killing of Adam Toledo. The 13-year-old boy was fatally shot by police despite having his hands up. "Prosecutors initially suggested Toledo had a gun in his hand when the officer shot him, but the state's attorney's office walked that back today," as Reason's Billy Binion reported yesterday. While Toledo did have a gun, "a slowly rendered version of the clip shows Toledo dropping a gun and raising both of his hands in the air, after which point he was shot in the chest."

The Adam Toledo video is horrifying. The kid didn't have a gun in his hand. And the state lied about it right up until they were forced to release the video.

    But the most glaring irony of all is that as Clinton-led Democrats this year equate overtures toward Russia as evidence of Putin-loving disloyalty — whether it be Trump’s opposition to arming Ukraine or his heretical questioning of NATO — there is an American politician who has, time and again, accommodated Putin, sought to improve relations with Moscow, dismissed as fearmongering the threat Russia poses to the U.S., and repeatedly taken steps that benefited Russian interests.

    That politician’s name is Barack Obama. As Trevor Timm wrote yesterday in The Guardian, “Barack Obama seems to be the only politician not playing into the cold war 2.0 hysteria.” Indeed, Obama has continually acted in accord with Russia’s agenda and sought to pour cold water on attempts to revive Cold War rhetoric and policies.

      Haven't watched the video myself but from what I've read elsewhere: Police see gun in hand. Suspect takes position next to fence with body turned partially concealing the arm holding the fun. He drops/tosses the gun behind the opaque fence where police officer can't really see it. Then starts raising his arms (which a reasonable person might interpret as raising to fire the weapon) and is shot.

      Would be tough to fault the officer in such a case if the details match.

        I dont fault the cop here, but if youre going to chase a guy with a gun, and tell him to stop and put his hands up, how would an armed person surrendering or turning to attack look different? There is problem with the whole procedure.

          Go see the video Talcum X linked in the main Toledo shooting thread, yesterday. When you do, you'll see that Toledo tried to spin his body, and thereby flick the pistol behind his back, so that it would go behind the fence he was standing next to. Some stage magician-type sleight of hand.

          Far better would be to just drop the gun, so the cop can see him do it. Also, keep Toledo's hands visible, visibly empty, away from his sides, remain still, and let the cop figure out what the cop wants to do.

          Of course, if he did that, then Toledo gets caught with a gun. Not a terribly big deal for someone on Toledo's career path, and at his age. He'd probably have been more worried about losing the gun, frankly.

            Presuming a show trial: Defense should have officers body camera video converted to VR headset and have each juror watch it in real time in VR.

        The full video compilation is worth watching. The kid pulls the gun out as he runs toward the gate, briefly holds it at his side so the officer can clearly see it being held in a shooting configuration, and then he rapidly spins the other way, obscuring the arm and hand with the gun as he spins. It is a very quick movement.

        The officer fires 0.8 seconds after the gun is flashed at the right hip.. after he spins rapidly. The gun hand is raising as he comes around and the officer's weapon discharges before the hand is perceptibly visible. He necessarily made the decision to fire based on the gun held in the shooting hand and the rapid spin toward him. It takes about .5 seconds to react and do something.
        They always trot out "split second decision". This is the first one where I do not have criticisms about the antecedent actions that created the split second decision.

        If this had been on a training course, 100 out of 100 people would have fired their weapon. It would be impossible to distinguish "shooting at me" from "surreptitiously flinging his gun and spinning toward me to put his hands up and surrender" given these circumstances.

        I blame the guy who shot someone and shoved the gun in a 13 year old kids hands.

        13 year old kid is 14 years old and quickly thrust into dangerous and potentially deadly situation against his will. Responds as a kid trying to avoid getting caught. You know, like a kid would. 21 year old murderer put him in that position.

        That is where your disdain belongs.

            Absolutely. Time to ask the good folks at Axon Enterprises (maker of Taser) if they have something cooking in the lab that can reliably neutralize threats without putting holes in them. Unfortunately, some people can shrug off Tasers so in a life or death situation they can't be relied upon.

            That, or creating the ranged equivalent of a truncheon: something that lets the subject know you are serious, but gentle enough to be deployed early in the force escalation. Right now, if the police aren't within striking range, their only options are gun, Taser, or yelling really loud. There needs to be something in between Taser and yelling.

          There is another aspect to the Toledo shooting that is being completely ignored. The cop in this instance did not give conflicting orders. He said "get your fucking hands up." He did not order him to turn around, to show his hands, or anything that would even remotely suggest that he should turn to the cop. Regardless of what the kid did with the gun, where his hands were when the shot was fired, or anything else, the spinning is what triggered the cop to shoot.

          The single shot was a completely appropriate response to a suspect with a gun that quickly and unexpectedly turns toward you. The fact that the kid got his hands all the way up before he fell over allows for that dramatic frame capture, but I agree based on what I see in the video that the decision to shoot happens before there is any opportunity to assess that he is no longer holding the weapon.

          This is a tragedy. Just not the one the media is furiously trying to paint. The guy that fired 8 shots and then tossed a kid the gun is 100% culpable here. Death during the commission of a felony.

        In real speed, I hesitate to say the cop shoulda coulda known his hand was empty. To me, this speaks more to our policies. Why was the kid being chased in the first place?

      This take is fundamentally dishonest, as was the earlier article in reason.

      Watching the full video from all sources, saying he "had his hands up", "was unarmed" or any other permutation is just a lie.

      Kid was victimized by his 21 year old buddy who shot someone multiple times at point blank range and then shiver the gun in the kids hands.

      Kid then flees police, refuses commands to stop, fiddles in his pocket, pulls out gun which is visible to the officer in his hand, then spins rapidly in an attempt to surreptitiously discard the fun through an opening in a privacy fence. As he spins and throws the gun (obscured from the officer's view) he raises his arms to about shoulder level and continues turning toward the officer. To any reasonable human being, this motion is identical to the motion he would be making were he about to shoot.

      This is a "stay off my side" moment. All of the folks who would like to see meaningful reforms in policing procedures and in legal structures and public policy regarding a police accountability should shun dishonest takes like the one reason pushed in their earlier article. By making fantastical claims and pointing at every shooting as equally emblematic of police brutality you discredit the entire movement.

      This is why nothing good happened in the wake of the Ferguson riots. People transformed a legitimate issue into a race baiting political tool. Sure, it gets activists on both sides all worked up. But it accomplishes nothing positive.

      You can both advocate for genuine reforms in police accountability both within their bureaucracy and within the legal system and acknowledge that this was an impossible situation, not some indication of systemic racism, trigger happy police shooting unarmed kids, or any other over the top interpretation. It is possible for two things to be true at the same time. Every police shooting is not an egregious violation of human rights, just as every incident of resisting arrest need not result in someone getting shot.

        I told Ken this yesterday, but these writers aren't interested in the meaningful discussion of reforming modern American policing. They're interested in promoting division, polarization, and ultimately the institution of a uniform, Federal police force to replace city, county, and state organizations.

        Which is a shame, as modern American policing needs reform in the worst way. But then again, it's only a reflection of the devolving society it serves.

          Watch the video again.

          Put yourself at .5 seconds before the officer's weapon fires. That is the moment you would have to think "shoot!" In order for your finger to pull the trigger 500ms later.

          Look at where the kid is. Look at where he is moving. Look at how rapid the motion is.

          If you claim from that motion in that moment that you can divine "he is putting his hands up and has discarded the gun" I am gonna call you out as a liar. You would have to be psychic to make that determination at that moment. At that moment he probably still has the gun in his hands at about belly button level and is in the act of flinging the gun... Which also looks exactly like pulling the gun up rapidly to fire.

          It ain't about "the kid deserved to die" vs "dirty racist cop executes people while on a power trip".

          It is about kid trying to avoid getting in trouble and thinking that this is a good way to do it at the same time that the officer sees someone with a gun immediately after multiple shots were fired and the guy with the gun refuses orders to stop and show his hands, fiddles in his pocket and produces a gun and then spins rapidly around to face him while raising his arms.

          That is the conflict. And the officer had zero ability to determine what the kid was actually doing, just as the kid did not perceive that the actions he was taking were about to get him killed. His "worst case scenario" was clearly "getting caught by the police with a gun".

          The officer's worst case scenario was getting killed by a fleeing criminal who just shot someone 8 times only moments earlier and who is turning toward him with a gun in his hand.

          If the kid had realized what the officer was experiencing at that moment, he would have gladly stopped and been arrested for having the gun. If the officer had known what the kid was up to, he would have gladly let him toss the gun and arrested him afterward.

          There have been a couple of studies done on black slaveowners in Maryland and South Carolina--basically, it was a way for black freedmen in the South to legitimize their place within society there, as you were less likely to be harassed if people saw you owned slaves.

    1) The report advocates breaking these companies up for tolerating "misinformation" among other things.

    2) The primary author of the report was Lina Khan, who has been nominated by Biden as commissioner at the FTC--which already has an antitrust case proceeding against Facebook.

    3) Whatever bipartisan basis there is of this is beside the point. The Democrats and the federal government are the same thing.

    Conclusion: Anyone who imagines that these companies targeting Republican speech on their platforms is somehow unrelated to the Democrats taking over control of the government is being willfully blind.

    When this Democrat plan to break-up BigTech companies by name because they tolerated "misinformation" was published in October of 2020, that was one thing when the Democrats were sharing power. Now that the Democratic party and the U.S. government are the same thing, protecting the speech of individuals from these private companies becomes a legitimate First Amendment issue.

    Show me a legitimate libertarian argument that it's okay for private companies to obliterate the speech rights of Republicans at the behest of a one-party, Democrat controlled government. I can't think of any situation at any point in history where defending a one party government intimidating private companies into suppressing the speech of critics of the party was the libertarian thing to do.

        The tech companies have learned by those who have grown a spine.

        In reaction to congressional hearings, they promised that they always provide a form of due process, telling people what action they are taking and why.

        The why has gotten them in trouble, because it is often a statement of fact.

        So they just skip that.

        Steven Crowder has some examples where his content has been taken down and his account locked for "Reason: ____________."

        If they claim "hate speech" or "offensive content" it is very difficult to argue, because those are opinions. But if they say "disinformation" then that is a statement of fact that can be libelous.

        Better to punt on the whole thing and just say "terms of service" or "community standards".

    Waaaay less than a second. 0.8 seconds from seeing the gun held at his hip to officer firing.

    In that 0.8 seconds the kids spins and tosses the gun in a manner designed to obscure that he is tossing the gun. It is probably 0.3 or 0.2 seconds from the moment he releases the gun to the moment the officer's weapon discharges.... But it is physically impossible for the officer to see this as it is obscured by the kids body and the fence (as was intended by the kid who was trying to avoid getting caught with the gun).

      I get what you're saying, but this isn't like that incident in New Mexico where the guy ambushed the cop with his rifle during a traffic stop. The cop already had the drop on this kid and had control of the situation. The kid was 13 years old, wasn't thinking clearly due to stress, and they would have found the gun with his fingerprints on it about five feet away where he tossed it.

        What he does know as a flat fact is that someone just fired several shots. He has the suspect running away. The suspect has a gun clearly held in his right hand, ready to be fired. The suspect spins rapidly toward him and raises his arms.

        That is all he can possibly know. That is why he shoots.

        If, in an alternate reality, that suspect had indeed been turning to shoot instead of turning to surrender while flinging the gun... There is little chance that the officer does not get shot. The suspect definitely gets at least one shot off.

      Which makes the promotion of ideas like raising the population of the US to a billion people, to say nothing of cramming them all in hyper-dense urban areas, even more absurd. Shoving more people into increasingly smaller spaces is not going stabilize or create high-trust societies.

    ENB raises the important issue of abortion, one of the policy issues on which I've long sided with pro choice Democrats, and a policy issue GOP leaders are still held hostage by Christian Fundamentalists and Conservatives, who want to ban all abortions.

    Ironically, while most Conservatives and Republicans oppose allowing more illegal immigrants into the country (especially since most will vote Democrat in the future if/when given the right to vote), conservatives in the GOP still want to ban all poor single women (who have the vast majority of abortions in the US, including blacks who have half of them) from having lawful abortions. And yet, most of those fetuses, if not aborted, will also vote Democrat in the future.

    But this is not really a racial issue, as poor whites and Hispanics account for the other half of fatherless children, who face similar economic and struggles as fatherless blacks.

    Conservatives are correct that the biggest problem facing the poor in America has been the breakup of families, especially fatherless children. So it makes no sense to ban single poor women from having abortions, as doing so will just increase the number of fatherless children, who are more likely to drop out of school, commit crimes, become single mothers and dead beat dads (and vote Democrat).

    Politically, had it not been for legal abortions during the past 5 decades, Trump wouldn't have won the election in 2016 (as Hillary would have got millions of more black votes because blacks would account for 16%-17% of the US population (instead of 13%).

    Now that Dems control the White House, House and Senate, they could/should be able to generate enough votes in Congress to enact federal legislation making Roe v Wade (or something similar) the law of land, which would also make abortions more accessible and affordable for poor women in Red States.

    But during two previous occasions when the Dems had control of the White House and both chambers of Congress (during the Clinton and Obama administrations), there were no efforts by Democrats (or liberal Democrat feminists) to make Roe v Wade into federal law.

    It appears that Democrats (and left wing feminists) would rather continue using Roe v Wade to oppose SCOTUS nominees by future Republican Presidents (instead of enacting legislation to ensure that women in all states have the right and access to abortions).

        I was a reason magazine subscriber for several years before I began posting online. I actually started posting on political sites at The Agitator. I started posting here because of the community there. (The Agitator was Balko's old website in the before times).

        There was a powerful thread of police reform and criminal justice reform running through the commentariate back then. Almost all of those people are long gone.

      Add it to the Burleson officer shooting a day or so ago, and the Mexia DPS trooper ambush a week before that. Or the officer near Bryan a week before that, who took a bullet in the eye, trying to stop that cabinet factory worker after the worker had shot up the place.

      Things are ripping apart.

        Yeah, it's not just that--gunmen at the San Antonio and Indianapolis airports (the former was shooting at people off of a bridge near Stone Oak before he went to the airport), and someone shot up a FedEx warehouse. Not to mention it's now rioting season again.

          Evidently someone tried to stop the FedEx shooter in the parking lot, and got killed for their pains. Sigh, at least they tried. No surprise that, per Heavy and CNN, the FBI had formally interviewed the shooter some time before his rampage.

          Well, at least he was "on your radar." Give them partial credit...
