FDA Finally Gets Around to Approving Pfizer's Anti-COVID Pill
It's another case of bureaucratic incompetence as the omicron wave surges.

The U.S. Food and Drug Administration (FDA) announced today that it has finally gotten around to issuing an emergency use authorization (EUA) for Pfizer's oral antiviral medication Paxlovid for the treatment of COVID-19.
Pfizer reported earlier this month that its clinical trial found that the medication "reduced risk of hospitalization or death by 89% (within three days of symptom onset) and 88% (within five days of symptom onset) compared to placebo; no deaths compared to placebo in non-hospitalized, high-risk adults with COVID-19." Paxlovid is a combination of the ritonavir protease HIV inhibitor and a new protease inhibitor that targets a specific enzyme that the coronavirus, including the omicron variant, needs to replicate and grow.
The FDA waited to issue an EUA until now, despite the fact that in early November, Pfizer stopped enrolling participants in its clinical trial because the treatment was so effective that it would have been unethical to give new participants a placebo. In the meantime, diagnosed COVID-19 cases grew by another 5 million and 50,000 Americans died of the illness.
The pills should only be taken by adults and children over age 12 and up who test positive for COVID-19 and are at high risk for progression to severe disease, including hospitalization or death. Treatment should be initiated as soon as possible after a COVID-19 diagnosis and within five days of symptom onset. Of course, the FDA's massive screw-up in delaying its approval of rapid at-home COVID-19 tests compounds the problem because many people will find it difficult to access the tests in time to let them know that they could benefit from taking Pfizer's pills.
Four weeks ago, the Biden administration paid Pfizer $5 billion for 10 million courses of the Paxlovid treatment. However, The New York Times is reporting that Pfizer is expected to deliver only enough of its pills to cover 65,000 Americans within the next week. As the Times notes, that is only sufficient at current infection rates to treat half of the people in the U.S. who test positive for the virus. Pfizer will be able to supply 200,000 courses of the treatment in January and 150,000 more in February. Still, every little bit helps.

The treatment is meant to be taken as 30 pills over five days. Patients take three pills at a time: two of Pfizer’s pills and one of a low-dose H.I.V. drug known as ritonavir, which helps Pfizer’s drug remain active in the body longer.

But people don't need it just because they "test positive". It's called for only for people in high-risk groups who have symptoms. They should quickly be able to produce enough for them.

Here's the big news of the day--far as I'm concerned: Two new studies came out about the hospitalization rate specific to the omicron variant.
The University of Edinburgh study, drawing on the health records of 5.4 million people in Scotland, found the risk of hospitalization with Covid-19 was two-thirds lower with Omicron than with Delta . . . .

The progressive argument for shutting down the local economy just morphed from only being defensible IF IF IF and when the local ICU became overwhelmed--to not really being defensible at all, since the hospitalization rate is significantly lower than other variants.
There remains some concern that the number of reinfected people could overwhelm the system--since our current vaccines are ineffective at preventing the transmission of omicron. But if the hospitalization rate associated with omicron infections were like it was with unvaccinated people, we'd be in serious trouble.
The argument against vaccine mandates remains the same--since the data at the moment suggests our current vaccines are ineffective at preventing the spread of the omicron variant (and the omicron variant is accounting for more than 70% of new cases). Why mandate a vaccine that's ineffective against the variant that's driving new cases?

The Scotland data is not really being compared well. From their own data - Scotland is now a population that is not only highly vaccinated but highly boosted as well. Basically 100% of the population over 60 has been vaxxed and 91% has been boosted. Even for 18-29 age group - 71% has been fully vaxxed and 20% is boosted.
Those numbers were not the same - in Scotland- during the earlier delta wave. So the reduction in hospitalization at this point in the omicron wave is more about reduced hospitalization in a vaxxed/boosted population than about reduced severity of the variant.
That said - even the South Africa data ( with much the same issue - a much higher vaxxed/exposed population now than in delta - plus MUCH lower age) is showing a very different pattern of hospitalization and death with omicron v earlier. Nowhere near the same ICU burden for treating covid in delta - far fewer deaths attributed to covid - but overall excess deaths and general hospitalization WITH omicron ARE increasing. Overall it appears more like omicron is enough to put already sick people back in hospital and be the final straw/complication for some.
Which is actually very good news.

Hopefully those trial numbers are correct, not like the Merck pill, which, when all the numbers were considered, did worse than the placebo at preventing hospitalizations. That didn't stop Trudeau from agreeing to send them $450 million to fatten their executive bonuses for 2022.
