The Biden Administration Will Release All Available COVID-19 Vaccine Doses Immediately
He will count on future production to provide second doses.

The U.S. is lagging woefully in its administration of COVID-19 vaccines as the pandemic worsens. At the current national rate of about 500,000 vaccinations per day, it would take 420 days to inject just the first of two vaccine doses into the shoulders of the nation's 210 million adults. The incoming Biden administration pledged to deliver 100 million doses in its first hundred days. But that's still way too slow because the new, more transmissible COVID-19 virus variant will greatly boost the number of cases, hospitalizations, and deaths if it is left unchecked.
Currently, the Trump administration is holding back about half of the current stock of 40 million vaccine doses to deliver as second doses for those who have already received the first inoculation. Vaccine makers Pfizer/BioNTech and Moderna are committed to producing 160 million doses over the next three months. Given those future supplies, American Enterprise Institute health care scholar James C. Capretta calculates that even if the pace of vaccinations jumps to 1 million per day, there would still be an excess of 86 million unused doses by the end of March. That is clearly unacceptable.
In the face of these considerations, the Biden team has decided to release all available doses for injection now. "The President-elect believes we must accelerate distribution of the vaccine while continuing to ensure the Americans who need it most get it as soon as possible. He supports releasing available doses immediately, and believes the government should stop holding back vaccine supply so we can get more shots in Americans' arms now," said TJ Ducklo, a spokesman for Biden's transition. Instead of waiting nearly two more weeks for the new team to implement this plan, Trump administration officials should immediately adopt this policy rather than clinging to their inflexible plan as the pandemic grows worse.
Former Food and Drug Administration (FDA) Commissioner Scott Gottlieb points out in The Wall Street Journal that vaccine production is speedily ramping up, so more of the current stock of 40 million doses should be used to inoculate people immediately since second doses would come from those new supplies. Gottlieb also argues that if stockpiles continue to build that the complicated vaccination program rules should be loosened and vaccinations should be offered to the general public based on age, which could be lowered from 75 to 65 if supplies allow.
"A vaccine that's sitting on a shelf for weeks, waiting for its perfect recipient, doesn't help snuff out the pandemic," notes Gottlieb. The good news is that the incoming Biden administration apparently recognizes this fact.

No doubt at all. Using all as soon as available is the only intelligent process. The second dose isn't used until two or four weeks later, depending on which vaccination. No point at all in holding back half all that time, especially when holding them back involves that expensive cold storage. Why double the storage requirements?

There are some valid and some invalid ways of trying to figure out what those study results might have looked like if the study had been different and had only used one dose. But that is always risky. It is not actually scientific. I think this is the right call.
It's a tough decision, though. The second does is key to getting fully immunized and it requires a pretty set timeframe for best maximization of the benefits. So, giving out second doses as first doses will risk upsetting maximum vaccine efficacy.
However, the countervailing argument of getting as many vaccinated as soon as possible adds the faster herd immunity situation, which I think has a good chance of outweighing the risk of failing to maximize the benefits with administration of that second dose possibly being impacted.
The second dose is not "key to getting fully immunized"; it's just the second layer. If the first dose is 80% effective (20% ineffective), two doses are 96% effective (4% ineffective), or something like that. It's not one does sits in wait and is 0% effective until the second dose activates it. The two doses are identical.
The time frames are just best guesses. One vaccine a two week interval, the other four weeks. Both probably work fine either way.

You just need to keep a rolling stock so you can give the second dose after the 30 days or whatever. The problem now is that we are not even giving the first dose fast enough. There is plenty of vaccine and more coming to have an effective program but the government is mucking it up as always.
I brought up that hospital in California. Their freezer went out and they only had 2 hours to use up their stock. They gave over 500 doses in two hours. They will get more and told people when to come back. This is not rocket science.

I agree - off the top of my head it sounds like it would be better to have 2X the number of people with only 80% immunity than to have 1X people with 95% immunity.

Wrong the second dose is not key. It is reported to be an incremental improvement. 80% is good enough if 150 million people get it in the winter. Combine that with the estimated 100 million or so folks who have already been exposed, then you are in the ball park for herd immunity.

Yes, it is key to MAXIMIZING the vaccine. First dose gets you in the 80% range, second dose gets you in the 95% range. That is literally maximizing the vaccine.
You need to look at the tradeoffs. If we had an infinite supply of vaccines then obviously everyone should get 2 doses. Since we don't have an infinite supply of vaccines then would it be better to have:
1. X people get 2 doses and 95% immunity
OR
2. 2X people get 1 dose and 80% immunity
I don't think it's a very obvious decision but just based on simple math then:
Case 1: 0.95X people are immune
Case 2: 1.6X people are immune
So just using back-of-the envelope calculations it sounds like case 2 (getting 1 dose to as many people as possible) would be the better choice to more quickly reach herd immunity and minimize death count.
That's exactly why my initial comment looked at the tradeoffs and explains that I agree with getting everyone at least the first shot as soon as possible.

I agree with your original post in this thread in which you have a rationale list of the pros / cons of 1-dose vs. 2-dose and come up with the conclusion that the proposed 1-dose strategy is probably the right decision for now. It's the health-care capacity that's important, not the statistics. When you run out of beds in hospitals, people with heart attacks, strokes and other non-COVID illnesses start dying un-necessarily.

California has had 2 million doses on hand for over a week and given out about 500,000. The doses arrived at places arranged between the Feds and the state months ago, as were the number of shots. The date depended on FDA approval, but the state was notified about a day in advance even then.

For the last two decades, China has been emitting more CO2 than any other single country/region on Earth and their increases have more than offset the US's reduction and, potentially any reduction the US could hope to attain. For about a decade, 'China' and 'The Rest of the World' account for more emissions and more of the increase than the US, EU, India, and Russia *combined*.
Ultimately I suspect Russia will be even more resistant to fighting climate change than China. Why would Russia possibly want to fight climate change? Most of Russia is bitterly cold tundra which will benefit greatly from global warming.

I think the idea is that the stock will be replenished in time to do the normal 2 dose regimen. Otherwise, you have huge numbers of dose sitting in the freezer for 21 days helping no one.

The problem is it chaotic and disorganized. There was no real plan in place. I mentioned Israel below. They have a highly efficient medical system. When you are always prepared to be attacked you have systems in place and a population who will come together.

I would argue that anyone over the age of 12 is capable. There are hundreds of thousands of junkies that can find a vein stoned out of their gordes and that's way harder than what it takes to deliver this vaccine. Between birth control, cholesterol, diabetes, and allergies, there have to be tens of millions of "qualified" sub-q injectors and that's before you get into their friends, relatives, and caretakers who know how to do the job.
Just showing that, once again, this virus is not an existential threat but an exercise in top down stupidity.

Unless the states get their act together on vaccinating their citizens, it doesn't matter how fast the Feds and the manufacturers ship the vaccine, it'll just sit around waiting for state governments to get their thumb out.
It is basically what Israel is doing and they are by far the world leader in this with 15-20% already vaccinated. They have ordered more and are planning a second dose with everyone over 16 vaccinated by the end of March.
At the rate we are going this is a total disaster. We had plenty of time to plan for this. The lockdowns, economic hardship, and suffering could go on for a very long time. 70% of the doses we have are still in the freezer.

The government effort was disorganized and haphazard. They did get orders in to the leading suppliers. That was good. They failed to get the doses where they needed to be. Too much red tape. Too much politics. You are fighting a war here and as the general said it is all about logistics.
