Kirsten Gillibrand Knows More Than Your Doctor When It Comes to Pain Pills
Putting the government at the center of health care means putting politics at the center of doctor-patient relationships.

Here's a tweet from Sen. Kirsten Gillibrand (D–N.Y.), who is running to win her party's presidential nomination. This should bother anyone who thinks that doctors should be able to treat patients without running everything past Congress. It's also a premonition of what to expect if a national single-payer health plan such as Medicare for All ever gets fully implemented (Gillibrand supports Medicare for All).
If we want to end the opioid epidemic, we must work to address the root causes of abuse. That's why @SenCoryGardner and I introduced legislation to limit opioid prescriptions for acute pain to 7 days. Because no one needs a month's supply for a wisdom tooth extraction.

Any number of things about this tweet and the legislation it's pushing are just terrible. The certitude with which Gillibrand speaks is terrifying, especially given her lack of medical training. That it's bipartisan underscores the notion that the only thing worse than a gridlocked government is one in which Democrats and Republicans actively work together (Cory Gardner is a Republican senator from Colorado).

And then there's the reality that "punishing patients won't reduce opioid deaths," as Reason's Jacob Sullum wrote in December, when a new Medicare rule lowered the amount of medicine that could be dispensed in a given prescription:
Opioid prescriptions, measured by total [morphine milligram equivalents], have fallen by a third since 2010, while opioid-related deaths have more than doubled. Instead of reducing deaths involving opioids, the crackdown on pain pills has pushed nonmedical users toward black-market substitutes, which are much more dangerous because their potency is highly variable and unpredictable.
The Trump administration wants to cut opioid prescriptions by another third in the next three years. What could go wrong? We already know.
As Sullum has documented again and again, restricting access to prescription painkillers has left patients in agony while pushing people with abuse issues into more dangerous and deadly situations.
Because of its size, Medicare already sets too many prices, regulations, and practices in the delivery of American medicine. If and when it expands to cover the entire population, you can expect not just a profound level of dysfunction based on how it already operates but ever-increasing political grandstanding along the lines of Gillibrand and Gardner's intervention into the doctor-patient relationship.

It is also known that the use of the derivatives is more addictive than the use of the opium plant. The War on Drugs is responsible for the suppression of the use of opium and the spike in the use of heroin. The high cost of contraband heroin led to the practice of intravenous injection in order to use smaller amounts to achieve the same effect and is exponentially more addictive than smoking or snorting.
Regardless, the idea that you can get addicted off one hit is pure poppycock. You couldn't ethically conduct the experiment it would take to reach such a conclusion based on the scientific method, and the anecdotes of addicts are spurious at best.
You might not want to believe the hype that the War on Drugs caused the problem with heroin, but I lived through the '80s and watched the government seize on the misuse of a diet pill (amphetamine) and manage to schedule-II that into the complete clusterfuck that is the meth epidemic.

I'd like to see her go through six months of Stage 4 throat and neck cancer treatment without opioids, as I just did. My daily OxyContin dose maxed at 250mg. I kept a journal of my dose and my calorie intake, in case anyone doubts my story. Surgery, chemo, and radiation (which burned my neck into a caricature of a grilled ribeye steak, ironic because I could only drink my meals). So who is going to say what 7 days of OxyContin is? 1,750 milligrams like me, or 560 milligrams that I started with? And who was going to go pick up the prescription every 7 days, and then go fill it? I was lucky to have a spouse, a daughter, and a sister who took turns driving me to radiation every day, and ensuring I got my medication. How many people are as lucky as I am? KG and others who espouse similar solutions have never faced the reality of pain that will kill you. I lost 50 lbs, and was on the edge of having a tube implanted in my stomach. All of this rhetorical nonsense demonstrates a complete lack of compassion for those facing what I faced. Does that mean I lack compassion for those who die from abusing opioids? Absolutely not. Surely there's an intelligent path towards mitigating abuse. But don't get in-between me and my doctor!
Cancer related pain is understood as a different category of pain. And usually, pain doctors understand this and will dose appropriately as it's a different disease/life process that the patients endure.
Opioids in general need to be prescribed less, as honestly, most of our country are bunch of fucking pussies. Many minor joint surgeries, general surgeries, and fractures that our nations's physicians prescribe a month of opioids (or more) for (because patients whine, bitch and complain if they dont get them), other countries give them some Tylenol and a handshake....and that's all they fucking need.
For clarification; govt of course shouldn't have any say whatsoever in this, as it is extremely variable on a case by case basis and needs to be evaluated by someone who knows what they're doing. Any limits they would try to set are an affront to the doctor-patient relationship and freedom in general....
I just think most doctors and patients need to grow a set on their own and learn to deal with minor to moderate pain, and treat extreme pain when it is merited.

Pain is subjective. Keep the decision between doctor and patient, and try not to judge. A broken rib or two plus an allergy that causes you to sneeze can be close to hell on earth for some people, but for others, it was just a fun night out getting in a brawl. Most ODs are from street drugs now, as the pill mills have been shut down. This is specifically about KG's one-size-fits-all thoughtless solution.

I wouldn't have to be denied them, I would reject the prescription up front as I have every time one was presented to me. Anti-inflammatories (OTC ibuprofen) were enough when I had broken ribs, broken collar bone, and massive skin abrasions everywhere when I got T-boned by a retard not paying attention in his pickup (I was on my bike). If I lived with stage 4 cancer and had mets everywhere, maybe that's a different story and I would take some.
As I said before, there are scenarios where it is appropriate but society as a whole needs to toughen up. I can't tell you how many people we see come through that are on adderall, ativan, and an opioid to get them through the day because they have been coddled and told "it's ok honey, you NEED those things, people just don't understand your truth, what you are going through". What we are really doing is crippling them because they don't learn any real coping mechanisms. Not to mention being on these things long term has other negative effects aside from the mental dependence.

The reason for this is usually because they are tolerant from...you guessed it...being given opioids too much for unnecessary things, and then being given them for an extended/chronic period of time without good reason. And no, there aren't millions of people walking around who just happen to be "resistant" to most pain meds other than opioids

I had spine surgery once. The doctor gave me a 30 day supply of something. I don't remember what it was. I took one pill and the whole world turned into a giant warm fuzzy blanket giving me a big friendly hug. When I came out of it I threw the rest in the toilet. I fear the drugs more than I fear the pain.
Later my wife mentioned those pills were worth $50 each on the street. I kind of wondered how she knew that, but I didn't ask.

Went down on my MC a few years back. Airlifted to hospital. Broken collar bone that is pinned together now. Collapsed lung. They stopped counting at 14 rib fractures. So many rib fractures my rib cage actually detached from my diaphragm (flail chest). About two weeks of limited movement developed kidney stones. In the words of the doc, 'easily identifiable kidney stones'. Eventually the stones needed to be blasted with ultrasonic to be broken down to pass.
A word on those 'evil' insurers .... I pay for my own insurance as I have the last 20 years due to the fact that I own an S Corp. The insurance company picked up the tab minus my $2500 deductible and has continued to grandfather me in so I can avoid all of the Obamacare bullshit. I love my private insurer.
